Clinical Scenario Number 1
  • Patient has IV catheter admitted in ICU setup
  • Organism came up Resistant to all cephalosporins / aminoglycosides / vancomycin
  • Gram positive, Catalase negative
  • Grows in bile, 6.5% NaCl , pH 9.6, 45 degree Celsius, 0.1% methylene blue
Clinical Scenario Number 2
  • Complaints of High grade fever > 40 degrees Celsius, Dry cough (on Ix - increased neutrophils, no organisms), Diarrhoea
  • On examination - Relative bradycardia, Neurological symptoms
  • Gram negative, Catalase and oxidase positive Resistant to beta lactams and aminoglycosides
  • Grow on Charcoal yeast medium
  • More - Facultative intracellular parasite, Causes fibrinopurulent pneumonia, Onset within 10 days of discharge from hospital. 

Clinical Scenario Number 3
  • By Animal exposure
  • Causes Non caseating granulomas
  • C/F - PUO / generalized lymphadenopathy / hepatomegaly
  • I/X - Catalase / Urease / Oxidase positive, Gram negative short rods
